A medical student who became our informant describes how medical institutions force patients to become involuntary victims of what would be more correctly called "medical rape." Standard forms of authorization for tests, signed by patients in a hospital, may also implicitly contain consent to unauthorized invasive methods of diagnostics of the innermost parts of the body while the patient is under anesthesia or unconscious.

In 2011, Shawn Barnes, a medical student from the United States, took the American Medical Association (AMA) approved standard course of study to become a M.D. and obtain a work license. The experience he gained in his third year of medical school haunted him for many more years. Sean was so worried about what had happened that he expressed his concerns to the administration of his university, where they were met with complete indifference. In order to expose the problem, he continued his efforts and wrote a separate paper that was published in 2012. What happened during his training in honey. institute, given the gold standard of American higher education, what made such a negative impression on the future doctor?

For Sean, who is now a physician, this has become a serious ethical and moral dilemma, which he reflects in this video interview, recorded during the filming of a documentary entitled "Your body at our service" / "At Your Cervix".

In this video, Sean explains that third-year students are required to undergo clinical practice in a variety of specialties, such as psychiatry, surgery, obstetrics and gynecology. During his 3-4 weeks of practice in the gynecological surgery department, Sean was asked to do something as part of his training, which he considered a gross violation of patient ethics and rights: he was given the task of conducting no less than 100 unauthorized gynecological examinations of women who were under anesthesia or unconscious, and did not give their consent for examination.

Sean talked about this to director Amy Jo Goddard, another free-thinker who now highlights this controversial way of teaching student interns. In discussing the extent and frequency of this practice, Sean and Amy estimated and estimated that only his fellow students could have performed approximately 6,000 unauthorized gynecological examinations. But Sean is not alone in his anger. A 2001 study at the University of Toronto found that about half of 108 medical students surveyed said that in the course of their clinical practice "They were pressured into unethical actions" and 61% said that witnessed unethical behavior of their clinical case manager... In addition, the students noted that “procedures for unconscious patients without their prior consent” are a serious moral issue, and yet the medical establishment remains largely resistant to change.

In Sean's case, when he challenged the ruling medical elite in his own university, in response to his allegations of violation of ethics, the administration only expressed mild bewilderment, clearly not wanting to consider ending the practice. which she considered extremely effective... The unstoppable Sean focused on legal changes and was successful in the early adoption of a law banning such practices in Hawaii. Such resistance to the system did not pass without a trace for him: due to his activity, John received a life-long denial of the right to live and work in Hawaii.

In addition to Hawaii, which recently agreed to ban unauthorized gynecological examinations, such examinations are also explicitly prohibited by the states: California, Illinois, Virginia and Oregon. But this practice continues to be legal in all of the remaining states.

Hospitals manage to obtain a kind of legal and informed consent from the patient, including in the standard set of documents when registering hospitalization the wording that students of medical institutes can be involved in the treatment and care of patients... Women who end up in hospital for gynecological surgery can become victims of this practice, regardless of whether the gynecological examination is listed in the list of procedures in their particular case or not. In states where this practice is not illegal, women awaiting gynecological procedures by signing standard forms, in fact, thereby waive their right not to participate in training procedures- observed or carried out by numerous students intimate and invasive diagnostics, while the patient is unconscious.

One of the most respected voices in the current debate belongs to Phoebe Friesen, an Oxford scientist who published a paper in 2018 entitled Gynecological Examination of Anesthetized Women by Student Trainees: Why Patient Consent Matters. This work gets to the heart of the matter: the lack of valid informed consent from the patient makes this practice unprovable and unforgivable. "Refusal of the patient's prior consent in medical practice can lead to desecration of the body, failure to respect basic human rights, and a breach of trust." Robin Fretwell Wilson of the University of Illinois describes the current standards of informed consent as nothing more than "modern medico-legal casuistry" in her work on "unauthorized practice" of teaching gynecological examinations for women under anesthesia.

The criminal law regarding unauthorized contact with the patient's body during medical examinations is replete with vague and outdated approaches, both in language and in the scale of the topic in general. Also, the laws differ from state to state. According to an article in the Journal of Ethics, the Journal of the American Medical Association, current laws in some states allow for recourse to those patients who feel abused. The Idaho Penal Code defines "acts of a sexual nature" as "inserting any part of the body or object into another person's vagina or anus without their consent" (Sec. 2907.01 (A)). The definitions of rape (Sec. 2907.02) and sexual harassment (Sec. 2907.03) include “the inability to resist sexual acts of the abuser due to the physical disability of the victim and / or her lack of consciousness that the corresponding act is being performed on her”. Gynecological examinations for women under anesthesia are consistent with all of these legal definitions, which can be useful in court hearings.

The problem of gynecological examinations is more global than individual incidents of unauthorized diagnostics of women under anesthesia. A recent study by the Journal of the American Medical Association shows that the results of gynecological examinations in women who do not complain of symptoms are inaccurate and far more harmful than beneficial. In other words, they are not based on real facts, but on the opinions of doctors, which is an outdated practice that, judging by the results of research, causes significant harm to women, including: “discomfort, anxiety, psychological problems, embarrassment and unnecessary procedures, including surgery (in 1.4% of cases, that is, for 29 out of 2000 examined women) ”.

In conclusion, please be careful when agreeing to standard surveys that are touted as “preventative” or “helpful” but that are not based on evidence. Also, please remember: do not transfer your rights to safety, protection and information to anyone while you are under anesthesia. Discuss this issue with your healthcare provider. Speak with a patient advocate at your hospital, or contact one through your insurance company, before undergoing gynecological surgery in any state that does not require separate, informed consent from the patient for instructed diagnostics by student interns. It may be worth talking to the elected MPs or putting your views in writing. In some states, citizens can leave their wishes for changes in legislation and create committees to protect their civil rights. The wave of change, demanding the abolition of this disgusting practice, has already risen. We can support her by spreading this message to local communities.

Another thing is that in some cases either large sizes or frankly unsuccessful models remained. But the latter is more of a taste.

No manipulations with price tags were found in DC Shoes and Ozone. The winter jackets that one of the journalists watched did become cheaper (but still expensive).

One of the largest online stores AliExpress opened the sale at 10:00 Moscow time. The site offers discounts up to 50%, as well as a $ 5 discount for orders over $ 30. But the price for the positions selected by the correspondent has indeed decreased.

At New Balance, the sale affected 170 men's products (out of a total of 594) and 146 women's products (out of 489). That is, even less than a third.

The Asos online store has completely abandoned Black Friday in Russia, to which its customers reacted extremely negatively, bombarding the company's page with angry comments. Cheating and price manipulation

The largest Russian online stores Lamoda and Wildberries prepared for Black Friday thoroughly and introduced a discount on thousands of products. Both sites started the sale early, and this turned out to be a monetary loss for potential buyers. Thus, during the "obstacle" in Lamoda, prices for a number of items became lower than during the sale itself on November 23rd. The same story happened with Wildberries. In this store, some items on Black Friday are now more expensive than a few months ago.

In the Citylink store, at first, journalists found quite advantageous offers and tempting discounts. But then the ideal picture began to crumble - for a number of products, the seller simply inflated prices several weeks before the big sale. So the Rosbalt correspondent was left without a washing machine and an electric kettle.

A similar situation arose at Eldorado. Two teapots of the Tefal brand, which a week ago cost 1 thousand 899 rubles, rose in price to 2 thousand 199 rubles on November 23. But next to the goods there was a sign “Black Friday”. An even more ugly story happened with a metal teapot of the same brand. It, as before, cost 1 thousand 999 rubles, but on "Black Friday" next to the price tag there was allegedly the previous price - 2 thousand 699 rubles. Here, the same tendency was revealed as at Citylink - a couple of weeks before the sale for a number of goods, the price increased. And closer to Black Friday, it fell, falling into the Black Friday section.

As a result. You can indeed find good deals on Black Friday, but you have to look well. In some cases, unscrupulous sellers mislead buyers with bright price tags and supposedly huge discounts. Therefore, there is only one advice: be vigilant and do not let yourself be deceived.
