Reasons for the formation of ptosis of the skin

Ptosis is the process of prolapse of body tissues. The reasons may be:

  • age-related changes in the body;
  • a consequence of hormonal disorders;
  • the consequences of liposuction, in which excess fat has been removed without tightening the skin.

Adipose tissue on the inner thigh is most often unevenly located. Its largest accumulations are most often concentrated in two areas: from above, closer to the inguinal fold, and from below, in the area above the knee.

Liposuction

This method allows you to remove adipose tissue, which is located under the skin of the inner surface of the thigh and creates coded folds, bumps, makes the skin visually and to the touch friable.

Limitations of the procedure

Don't expect a miracle. This correction method has its limitations, which should be taken into account when you plan to improve your appearance with this.

  • The surgeon cannot lose weight for you.

This is the most important limitation of the method, which patients prefer not to know, and which plastic surgeons do not like to talk about. Mandatory list of examinations

To find out the presence or absence of contraindications to the operation, you must perform:

Procedure execution

Regardless of which of the areas is supposed to be treated, the one closer to the groin fold, or the one closer to the knee, the skin is punctured in the popliteal fossa. The popliteal fossa region is characterized by a high concentration of large blood vessels and nerve trunks. Therefore, manipulations in this area require caution.

It is performed under general anesthesia. It takes about an hour. When the entire volume of excess fat has been removed, the skin puncture is treated with an antiseptic and sealed with an adhesive plaster.

Recovery period

Immediately after the completion of the operation, a compression garment is put on the patient. You will need to wear it for at least 2-3 weeks.

Target:

Complications

  • Decrease or complete disappearance of skin sensitivity at the site of the operation.

As a rule, such a change in sensitivity is not persistent and over time the sensitivity of the skin is restored.

  • Damage to superficial veins.
Most often, the small branches extending from the great saphenous vein of the thigh are damaged. It is almost impossible to completely exclude such a complication, since the structure and location of veins is more variable than the location of arteries and other anatomical structures.

If a vein is damaged, it is tied or closed with a clip and the operation is continued. Venous outflow in this case occurs along the nearby veins.

  • The appearance of increased skin sensitivity.

Hyperesthesia occurs in about 1% of all liposuction patients. The severity of unpleasant sensations can be of varying degrees. In some patients, skin sensitivity persists for life.

  1. Chronic pain at the liposuction site.
  2. Death of the skin.
  3. The appearance or increase in the severity of ptosis or flabbiness of the skin.

It happens when the skin cannot contract enough to ensure sufficient smoothness and elasticity of the thighs.

  • Violation of lymph outflow with the formation of persistent edema of the leg and foot.

A large number of lymphatic vessels pass under the skin of the thighs in the thickness of adipose tissue, through which lymph flows to the body from the entire leg.

If, during liposuction, the outflow of lymph is disturbed, then the tissue fluid begins to accumulate in the area first of the foot, and then the foot and lower leg and forms soft edema.

Usually, the lymphatic drainage is gradually restored and the swelling goes away.

But in some cases, a violation of the outflow of lymph can become chronic, which will ultimately lead to the formation of elephantiasis (elephantiasis or pronounced edema of the legs).

  • Anemia.

It can develop if the volume of adipose tissue removed is large. Removal of adipose tissue is accompanied by bleeding, which in some cases can be very intense.

  • Fat embolism.

It can develop in cases where liposuction is performed simultaneously with lifting the skin of the thighs or the anterior abdominal wall. In this case, adipose tissue enters the bloodstream and can block the lumen of the vessel, disrupting the blood supply to the tissue site.

  • Changes in skin color and postoperative scars.

Skin pigmentation at the liposuction site may develop for several months after surgery. Hyperpigmentation can be persistent, requiring laser or phototherapy to remove it.

  • The effect of the "washboard".

A feature of the location of the fatty layers of the thighs, abdomen and chin is that the fat in these areas is located in layers, which are separated by fascia - connective tissue formations that look like thin films. Those layers of adipose tissue, which are located in deeper layers, differ in metabolic rate, in which fat accumulates quickly and leaves extremely slowly, even despite a significant decrease in body weight.

This feature is genetically determined. That is why very often these areas are called "fat traps".

At the same time, the presence of connective tissue layers requires the surgeon to perform the procedure correctly. Since the removal of excess fat at different levels can have the opposite effect: instead of eliminating a cosmetic defect, liposuction will create another one that is even more noticeable.

If the "washboard" effect is still formed, then a second liposuction will be required, which not every plastic surgeon is able to perform in order to correct the "washboard" effect and give the legs the long-awaited smoothness. In some cases, it is no longer possible to fix the defect.

How is the operation going

Preparatory stage

The operation is usually performed on the day the patient is admitted to the hospital. Complications of femoroplasty

  • The appearance of rough scars.

The most common reason for the development of such scars in the case of tissue fixation to the pelvic bones is friction. In both cases, small wounds can develop in the areas of greatest friction, which, when healed, create irregularities in the scar. This problem is solved in consultation with a surgeon, who prescribes dressings with healing ointments or the imposition of secondary intradermal sutures.

  • Development of marginal necrosis (necrosis) of the skin.

The blood supply to the skin in the perineal region is relatively weak. And the tension on the edges of the skin can be very significant. Under such conditions, the blood supply to the marginal areas of the skin that form the postoperative scar may be impaired. Areas of the skin in which the blood supply is disrupted die off. Traditional techniques and their general characteristics

  3. A spiral incision that runs from the gluteal fold to the groin fold.
  4. Plastic surgery in a combined way.

Compared to other plastic surgeries, hip plastics until recently were performed reluctantly by surgeons, and patients were often unhappy with the results. This is due to the fact that operations in accordance with classical methods led to multiple complications and required a very long rehabilitation period.

For example, when excising the skin in the groin area, where a large number of lymphatic vessels and nodes are located, it was impossible to excise part of the tissue without damaging the lymphatic ducts. As a result, lymphostasis (stagnation and accumulation of lymph) and associated pronounced edema of the extremities subsequently developed, often prolonged (up to 2 months) lymphorrhea (lymph flow) into the area of ​​skin detachment, infection of this area and dangerous suppuration up to a septic state ... The problem of lymphorrhea is not solved by ligation of a specific lymphatic vessel, as with bleeding, or with the help of medications.

The postoperative period was usually not only long, but also difficult for patients. In addition, due to the constant functioning of muscles and joints and a decrease in tissue elasticity, postoperative scars are stretched and gradually displaced from the femoral-perineal region to the region of the anterior and medial surface of the thigh. Resulting in:

  • scars appear in the area uncovered by linen and become visible;
  • there is an expansion of the perineal region until the femoral-perineal grooves are completely smoothed out.

All this forced the majority of surgeons to refuse to carry out such operations.

Improvements in thigh lift techniques

Various modifications of traditional methods have emerged over the past few years. The deformation of the thighs occurs along their inner and outer surfaces. However, lifting the outer surface of the thighs does not cause any particular difficulties, since the skin here stretches much less. Therefore, in most cases, only liposuction is enough, and possibly additional excision of a small area of ​​the skin. In addition, large lymphatic vessels are not located in this zone.

It is much more difficult to lift the inner thighs, in the area of ​​which fat deposition and tissue changes are expressed to a much greater extent than in other parts. However, some changes and significant additions to the classical methods of performing the operation allowed:

  1. Reduce the risk of damage to the integrity of the lymphatic vessels.
  2. Prevent displacement of postoperative scars.
  3. Significantly improve the results and portability of surgeries.
  4. To shorten the time and facilitate the course of the rehabilitation period: the very next day the patient has the opportunity to return home and almost limit himself to nothing.

The essence of the improved lifting techniques is to make an incision 20-40 cm long (depending on the volume of the thigh) in the area of ​​the groin-pubic fold. It runs along the edge of the pubic bone, and the subsequently formed scar is easily covered with underwear.

After liposuction, excess tissues are not excised, not removed, but, unlike standard classical methods, are separated (split), which prevents damage to the lymphatic system and, therefore, prevents the occurrence of complications such as lymphostasis or lymphorrhea.

Then the skin is tightened and redistributed to the place freed from fatty accumulations. The tightened lower skin flap is sutured not to the muscle fascia with multi-row sutures (it is dense, but nevertheless, in the future, under the influence of muscle loads, it is subjected to stretching), but to the pubic bone periosteum. This makes it possible to reduce the load on the postoperative scar, minimize wound complications and negative consequences in the form of scar displacement.

The solution to the problems of hip contour correction is very difficult, and its results are ambiguous. In plastic surgeries, it is important not only to correctly perform them technically by a plastic surgeon, but also to be able to understand what kind of incision needs to be made, how much fat to remove, how far it is necessary and possible to move the skin.

Unfortunately, modifications of the classical methods of soft tissue tightening have not yet become widespread and are performed by not many surgeons. New techniques have further complicated the implementation of plastics and increased the duration of its implementation to 4 or more hours, but they have largely avoided serious complications and increased the effectiveness of the operation.
